Our Pharmacist works as part of the practice team to resolve day-to-day medicine issues and consult with and treat patients directly.

The role of the clinical pharmacist includes:

    • Offering medication reviews for patients with long-term conditions such as asthma and hypertension (high blood pressure),or patients on multiple medications.

    • Seeing new patients who take a number of medicines to ensure that their current medication and appropriate medical follow-up is planned.

    • Dealing with patient prescription queries and offering telephone consultations with patients who have a medication related query.

    • Ensuring changes made to your medication following a hospital admission or out-patient visit are updated on your repeat prescription.

You can book a face to face or telephone appointment with the pharmacist if you would like any information regarding your current medications.
